If you are a person or business in the United States, its territories, and the District of Columbia who purchased, for personal use, any Covered Product, you may submit a claim for a Cash Award from this class action Settlement. Claim Forms must be submitted online or postmarked by December 27, 2022. Claim Forms received after the deadline will be deemed untimely and may not be accepted.

Settlement Class Members may submit a Claim Form to receive 25% of the average retail purchase price, up to $100, for purchases of fairlife Milk Products and FOF Milk Products, if the products were purchased for personal use and not for resale, and were purchased on or before April 27, 2022. The list of the Covered Products is available here.

Claim Forms submitted without Valid Proof of Purchase will be capped at a Cash Award of up to $20 and Claim Forms submitted with Valid Proof of Purchase will be capped at a Cash Award of up to $80, subject to certain adjustments upward or downward depending on the number of claims submitted.

  • Claims without Valid Proof of Purchase: Claimants who submit a Claim Form without Valid Proof of Purchase will be eligible to receive a maximum of $20 as a Cash Award, subject to an adjustment upward or downward, based upon the number of claims submitted.
  • Claims with Valid Proof of Purchase: Claimants who submit a Claim Form with Valid Proof of Purchase will be eligible to receive a maximum of $80 as a Cash Award, subject to an adjustment upward or downward, based upon the number of claims submitted.
  • Claims with AND without Valid Proof of Purchase: Claim Forms submitted with and without Valid Proof of Purchase are allowed, and Claimants who submit such Claim Forms will be eligible to receive a maximum of $100 as a Cash Award, subject to an adjustment upward or downward, based upon the number of claims submitted. Note: if you submit Valid Proof of Purchase for all claims, you are still eligible to receive a maximum of $100 as a Cash Award, which may be adjusted upward or downward.

"Valid Proof of Purchase" means verifiable documentation of a transaction that reflects the purchase of one or more Covered Products on or before April 27, 2022. Examples may include but are not limited to store receipts, milk bottles, or any other contemporaneous record of purchase that is objectively verifiable.

Claims are limited to one Claim Form per household.

How to File Online:

To file online, you will be asked to provide contact information and purchase information for fairlife and FOF Milk Products you purchased, including type, size, and quantity. If you are submitting a claim with Valid Proof of Purchase, please have your documentation ready before you start to file, as you will not be able to save your progress and return later to finish. Documentation that is uploaded must be less than 20 MB per file and in one of these formats: jpg, jpeg, png, gif, tif, tiff, doc, docx, xls, xlsx, pdf, txt, rtf, or zip. Supporting documentation (if submitted) should be clear, readable copies, as they will not be returned to you. If you are unable to upload your supporting documentation as part of the online Claim Form, you will need to mail a printed Claim Form along with your documentation to the Claims Administrator.

After submitting your completed claim online, you will receive an email with a confirmation code for your completed submission. Be sure to keep your confirmation email and code and refer back to them if you have any questions about your Claim Form.

How to File by Mail:

To file by mail, you may download a copy of the paper Claim Form here or request a copy be mailed to you by calling the Claims Administrator at 1-855-604-1865 (Toll-Free). Then, type or legibly print all requested information, sign, and date it. Mail your completed Claim Form, including any supporting documentation to the the Claims Administrator at the following address:

Upon receiving a completed Claim Form, the Claims Administrator will review Claim Form and the documentation (if applicable) to confirm or deny your eligibility for a Cash Award. If you fail to provide required documentation for purchases in excess of $80.00, you may receive a Deficiency Notification by email or US Mail if you have not provided a valid email address.

Please keep copies of everything you submit, as your claim may be audited, and you could be asked to provide additional information to process your claim.
